Band 7 Children's CHC Nurse Assessor We are currently recruiting for an experienced Band 7 Children's CHC Nurse Assessor to support an NHS Integrated Care Board in London. This is an excellent opportunity for an experienced Continuing Care professional to join a busy team responsible for assessing, reviewing and coordinating care packages for children and young people with complex health needs. Key Responsibilities: * Undertake Children's Continuing Care assessments and reviews in line with the National Framework and local policies. * Assess the healthcare needs of children and young people with complex needs. * Produce high quality assessment reports and recommendations. * Participate in multidisciplinary team meetings and case discussions. * Work collaboratively with families, healthcare professionals, education providers and local authority colleagues. * Support care planning and ongoing review processes. * Ensure accurate and timely documentation is maintained. Essential Requirements: * Registered Nurse with current NMC registration. * Previous experience undertaking Children's Continuing Care (CHC) assessments. * Strong understanding of Continuing Care processes and frameworks. * Experience working within NHS services. * Excellent communication, report writing and stakeholder management skills. * Ability to work autonomously and manage a varied caseload.
