The ideal candidate will work as an Early Help Family Practitioner in accordance with the Family Support Practice Guidance and Multi-Agency Team protocol. The role will involve: To promote an environment that safeguards and protects children and takes responsibility for ensuring that appropriate safeguarding procedures are adhered to. To provide structured and individual support to children and families using the appropriate assessment tools, such as Early Help Assessment Form and, to create an individual plan with agreed outcomes, targets and review dates. To act as lead professional for individual families to coordinate and share information at MAT Meetings and to support and lead on team around the child meetings To support parents/carers care of their child through targeted work in the home, group work and coordinated services and activities; and support with barriers such as housing and financial matters that impact on parenting.
