The purpose of the role is to manage a case load of youth justice cases, working innovatively with young people from diverse backgrounds and engaging them and their families and other professionals in order to assess, develop and deliver intervention and activities that support their personal and social skills and participation in society YOS Officers will be directly contributing to youth offending service outcomes by creatively engaging young people who are subject to youth justice orders and also who are at risk of anti-social behaviour, crime, becoming NEET and substance misuse. This job description is intended as a guide and is not an exhaustive list of the duties and responsibilities of this role, such duties may vary from time to time without changing the general character of the post or the level of responsibility entailed. To effectively engage with young people from diverse backgrounds at all stages of case management including during assessment, review and delivery of interventions/programmes. To gather information, analyse and assess in order to manage risk, re-offending and safety/wellbeing accurately using appropriate screening and assessment tools; taking necessary measures to ensure welfare needs are dealt with effectively and public protection considerations are appropriately managed. To create, in conjunction with young people and families, bespoke intervention plans to address all areas of need and to regularly review the plans to ensure their suitability and effectiveness. To work within a multi-disciplinary team, to be responsible for all aspects of case management of youth justice cases, including dealing with non-compliance issues swiftly and effectively in line with local and national standards and inspection frameworks To prepare for and attend professional meetings such as strategy meetings and child protection conferences, presenting information in a clear and concise manner. To write reports to a high standard and within tight timeframes for Youth Magistrates and Crown Courts and to confidently present these reports to the Court in person as required. To undertake regular Court duty, providing advice and support to young people, their families and the judiciary. Undertaking assessments at Court will also be part of the role when working in court. To undertake bail assessments and produce bail support packages for court. Evidence of effectively managing high risk and complex cases and assisting with the day to day support of less experienced YOS staff. Lead responsibility for liaison with key agency e.g. CAMHS, Health Visitors, Schools as directed by the Team Manager. To convene and chair multi-agency meetings. To play a leading role in improving practice and quality of assessments and plans, assisting Team Manager in Team meeting discussions and workshops, taking responsibility for updating Team on practice developments and research findings, take a lead role in the induction of new staff and being a buddy. To act as lead professional for allocated cases and work collaboratively with partners using a family based approach to work towards increasing young people and family resilience. Working collaboratively with young people and a wide range of agencies, adopt creative and innovative approaches to deliver effective evidence based group or individual interventions/programmes to reduce the risk of offending and disengagement from education, training and employment To adopt a restorative approach in engaging young people and work together with colleagues to support opportunities to repair harm. To maintain clear, precise and appropriate case management records on IT client information systems, in accordance with local and national standards and in line with data protection guidelines Contribute to the development of team/project/service plans and procedures to ensure these reflect the needs of young people. Prepare, deliver and participate in the running of group work programmes This role requires flexibility in order to meet fixed deadlines and competing priorities. The role will require regular attendance at secure estates, Police stations and community facilities. To attend all team meetings, case manager/practitioner meetings and other meetings internal and external which are relevant to the role, eg: Risk Management Panel and MAPP Home visiting is a regular requirement of work with families, at times outside 9-5 hours to avoid interfering with school and work commitments of families. The public engagement element this role involves regularly coming into contact with people, some of whom may at times be distressed, agitated and, from time to time challenging. The post holder will be required to use their initiative to complete certain tasks, with supervision. Creativity is required for identifying, developing and delivering appropriate interventions for young people. The post holder will need to work flexibly across levels of need (from lower risk community orders to high risk custodial cases) Carry out all duties in accordance with the London Borough of Lambeth s Equal Opportunities policy, actively promoting equality and seeking to prevent and overcome disadvantage and discrimination. Job Purpose To co-ordinate the statutory process for Education, Health and Care (EHC) Needs assessments for children and young people with special educational needs and disabilities, across the age range of 0-25. Ensuring that all statutory timeframes in the assessment process are met and delivered on time, as well as writing and co-producing Education, Health and Care Plans with children and their families. Responsible for a case load of children and young people that have EHCP s and ensuring that the needs of these children and young people are appropriately met in their provision. This includes ensuring they have appropriate support, resources and placements at early years settings, schools and colleges to enable them to achieve their maximum potential for learning. To assist the EHC Team Manager, the EHC Area Lead s and the Senior EHC Co-ordinator in the delivery of efficient systems to ensure that statutory processes are carried out within required timelines, and information is recorded accurately on the LA s database. To support the statutory annual review process of Education, Health & Care plans and attend person centred review meetings at key transition points. Specific Accountabilities of the Role Undertake statutory needs led EHC assessments in accordance with the Children and Families Act 2014, and the SEND Code of Practice 2015, in conjunction with contributions from children, young people and their families, and other professionals across education, health and social care as appropriate within statutory timescales. To plan and facilitate person centred multi-agency meetings, including acting as chairperson at meetings that agree a draft EHC Plan and indicative personal budget, if appropriate. Construct and develop EHC Plans from approved assessments to achieve stated objectives and outcomes in relation to the needs of individuals, reflecting as closely as possible service users aspirations. Construct and develop Multi-Agency Plan (MAP) of Support if assessment determines support should be provided from universal and targeted services. As the named Coordinator, to be the main point of contact for all service users, including families and schools, ensuring their full involvement in meetings and development of the EHC Plan. To work closely with all professionals, including Key Workers, to ensure their knowledge and experience informs the EHC plan, or MAP of support. To provide written briefs and oral representation to the EHC and other Panels as required. Undertake the on-going co-ordination, monitoring and annual review of EHC Plans to ensure the service user is receiving the appropriate level of support for their identified needs agreeing to maintain, amend or discontinue of the Plan as appropriate. To attend Key Statutory Annual Reviews for pupils with EHC plans providing advice on SEN issues and provision. Maintain and update all relevant records and forms in accordance with the General Data Protection Regulation and Data Protection Act 2018 and London Borough of Barking and Dagenham policies and procedures for effective working, efficient audit and clarity of information for management relating to performance targets and service provision. Housing (Homelessness) Triage/First contact Officer Office based 36 Hours per week (flexibility for hybrid working considered once role established) To provide an effective triage and duty service and conduct a pre assessment/filtering service by telephone or face to face interviews to provide advice and assistance for those in housing need or are threatened with homelessness or are homeless in accordance with the Homelessness Act 2017. Duties Interview and assess homeless on the day approaches in person. Make preliminary investigations ensuring customer provides evidence and documentation at the earliest opportunity. Provide comprehensive advice on a full range of options to solve their housing problem including tenancy rights, housing, and welfare benefits. Makes initial enquiries into reason to believe client is eligible and homeless. Filters and contacts online housing options form and conducts preliminary enquiries, providing advice and assistance to prevent homelessness occurring. Checking Section 21 validity and Refer to Safer renting regarding illegal evictions. Undertake email duty and respond to emails or forward to the most appropriate service/team Undertake telephone duty provide housing advice or advise of the relevant service/team details. Respond to Duty to Refer and make initial enquiries and first contact. Capture and process data on the Council s housing system (NPS) to accurately reflect the application process and stage of application, creating new application records as necessary. Ensure accurate and comprehensive case notes and records are kept of contact and advice given. Ensure documents are scanned and labelled correctly and added to the customers records. If homelessness cannot be prevented or client is homeless referred to the Housing Duty Manager for allocation to a case officer and requesting temporary accommodation.
