Senior Social Worker - Adults - Hospital Team

Ackerman Pierce is currently supporting a leading local authority within Windsor & Maidenhead to recruit for a Senior Social Worker in the Hospital Discharge Team within the Social Care industry. This will be on a permanent basis paying 45,831 - 50,414 per annum plus 3,000 annual retention bonus.

Responsibilities include:

• Working with Vulnerable Adults within a hospital setting
• Report writing and making assessments
• Developing care plans
• Care management
• Managing complex cases
• Liaising with other professional bodies
• Arranging safe and timely discharges for service users
• Self-directed support / personal budgets

• Supervise Social Workers and Case Management Officers
• Handle complex cases, including MCA Assessments, Court of Protection applications, and s42 safeguarding enquiries

Experience preferred:

• Post qualifying experience of working as a Senior Social Worker - Hospital Discharge team within the UK Social Care industry.

Minimum Requirements:

• HCPC Registration
• 1 year post qualifying experience within UK Social Care industry
• Enhanced DBS/CRB (we can obtain for you)
• Eligibility to work in the UK
