Qualified Social Worker - Adults Hospital Discharge Location: West Yorkshire Pay Rate: £35 per hour Contract Type: Locum (Flexible Agency Role)Are you an experienced and passionate Social Worker with a background in hospital discharge? We're working with a local authority in West Yorkshire seeking skilled Qualified Social Workers to join their Hospital Discharge and Home First Team. This is a fantastic opportunity to support people transitioning from hospital to home, while working in a fast-paced, multidisciplinary environment.Take the next step in your social work career while making a real difference to those recovering from hospital care. ? About the RoleAs part of the Hospital Discharge Team, you'll help facilitate timely, safe discharges from acute settings - ensuring the right care and support is in place from day one.You will:
  • Manage a caseload of adults with complex needs
  • Carry out strengths-based assessments and create tailored care and support plans
  • Deliver statutory responsibilities, including Care Act assessments, reviews, and safeguarding interventions
  • Work closely with health professionals, therapy teams, discharge coordinators, and care providers
  • Prioritise early intervention, recovery, and independence
  • Participate in a duty rota (1 week in every 4)
