As Clinical Package Manager, you will work closely with Team Leaders and frontline staff to ensure safe and effective care delivery within the person's own home, in the local community, and during any hospital stays. Your role will involve clinical leadership, hands-on care, and collaboration with families and healthcare professionals to ensure the best outcomes for the person we support.
Key Responsibilities:
Deliver and coordinate person-centred care, ensuring dignity, choice, and independence
Provide direct clinical care as needed, including occasional weekends
Develop and review clinical care plans and ensure they are implemented effectively
Oversee medication management in line with current legislation
Lead on infection prevention and hygiene practices within the package
Ensure compliance with Care Quality Commission (CQC) standards and internal policies
Collaborate with healthcare professionals to meet clinical needs effectively
Support and guide Personal Assistants and Nurses, ensuring quality care delivery
What We're Looking For:
A qualified healthcare professional (e.g., RN/RGN) with relevant experience
Strong leadership and clinical assessment skills
Experience in community or home-based care (preferred)
A person-centred approach with a commitment to dignity and respect
Excellent communication and organisational skills
A proactive, caring, and values-driven attitude
