Social Prescribing Link Worker

Are you passionate about helping people improve their wellbeing and create positive change in their lives? We're looking for a motivated and compassionate Social

Prescribing Link Worker to join our Greater Middlesbrough Social Prescribing Team, with a dedicated focus on supporting people experiencing complex mental health needs.

Working across GP practices within the Greater Middlesbrough Primary Care Network, you'll provide personalised, holistic, and trauma informed support to individuals who may face barriers to accessing services or experience health inequalities.

In this rewarding role, you will:

• Work one to one with people to explore their strengths, goals and the challenges they face.
• Connect individuals with community groups, activities, and services that enhance their wellbeing, confidence and social connections.
• Support people to develop coping strategies, improve emotional resilience and navigate difficult circumstances.
• Work in partnership with primary care and community organisations to ensure the best outcomes for those you support.
• Help identify gaps in local provision and contribute to the shaping of future services.

You'll need to be caring, enthusiastic and highly organised, with experience of working in trauma informed ways and supporting people with complex or multiple needs. Strong communication skills, the ability to work independently, and a commitment to empowering individuals are essential.
