Cheshire East - QSW - Hospital discharge

We are currently recruiting for a Qualified Social Worker to join the Hospital Discharge Team in Cheshire East. This is a ward-based role at Macclesfield Hospital, supporting timely and safe discharges for patients on Ward 9.

This is a fast-paced role ideal for experienced social workers with strong assessment and discharge planning skills.

Job Title: Qualified Social Worker - Hospital Discharge
Location: Cheshire East (Macclesfield Hospital - Ward Based)
Working Hours: 8:30 AM - 5:00 PM (On-site, 5 days per week)

Key Responsibilities:

• Undertake Care Act assessments to support hospital discharge

• Work closely with NHS staff, MDTs, and external agencies

• Facilitate safe and timely discharges from hospital

• Arrange care packages, placements, and support services

• Manage a caseload of patients on Ward 9

• Ensure compliance with discharge pathways and legislation

Requirements:

• Qualified Social Worker with Social Work England registration

• Previous experience in hospital discharge / acute settings

• Strong knowledge of the Care Act and discharge pathways

• Ability to work in a fast-paced, on-site hospital environment

• Excellent communication and multi-agency working skills

Additional Information:

• Must be based on-site at Macclesfield Hospital every day

• Working hours are 8:30 AM - 5:00 PM

• Focus on Ward 9 patient discharge assessments
