Reach Out is a volunteer project that provides companionship and practical support to older people who are medically vulnerable and at risk of hospital admission, or have recently been discharged from hospital. Volunteers spend up to one hour each week visiting clients in their homes to offer companionship, a cup of tea, conversation, short walks, and help them recover and reduce isolation. Responsibilities Visit older clients at their homes up to one hour each week Provide companionship through conversation, tea, and short walks Assist with practical support to help them recover from illness or discharge Help reduce social isolation and loneliness Qualifications Friendly and caring Committed to spending one hour per week on visits Able to visit clients in their own homes About Reach Out Hertfordshire Reach Out links older people with volunteers in their neighbourhood who can offer support in their own home - helping them to adjust to and recover from illness, whilst also reducing social isolation and loneliness.
