This particular service supports individuals to actively progress along their mental health recovery journey through support planning, proactive key working, the delivery of structured courses, therapeutic activities and signposting to appropriate services. Creative Support is a non-profit organisation which provides high quality person centred social care services for people with learning disabilities, mental health and other needs. We are looking for person centred individuals who are well motivated, hardworking and keen to join our friendly team. You will work with a caseload of service users to develop individual recovery plans and coordinate a package of support which will enhance their quality of life and provide opportunities for greater social inclusion within the wider community. You will engage with service users to build trusting, therapeutic relationships and use a psychologically minded approach whilst maintaining professional boundaries and working within Creative Support's Recovery Model to encourage positive mental health and coping skills. Vacancy reference number - 93236 Our timetable of structured courses and therapeutic activities is updated every three months and new or alternative sessions will always be considered. We have a particular interest in applicants who have experience delivering Mental Health Management, Psycho-education and Positive Lifestyle themed sessions. This role offers unique and challenging opportunities bringing great job satisfaction to those who are looking to make a positive difference to the lives of others. We provide relevant training including accredited health and social care diplomas through our own training academy. This is a role which is available for people who are committed to making a positive difference, including experienced care workers, graduates and people looking for a stimulating career path into social care. Working with a person-centred and recovery-focused approach, you will manage a caseload of service users, helping individuals to identify their goals, develop practical support plans, manage risks, and build the skills needed to live as independently as possible. You will work collaboratively with internal teams, families, healthcare professionals, and external agencies to ensure service users receive safe, effective, and meaningful support. Key Responsibilities: Manage a caseload of service users, providing personalised, flexible, and outcome-focused support. Complete assessments, support plans, task plans, and risk management plans to promote independence and wellbeing. Support individuals to achieve personal goals, develop life skills, and access opportunities within their local community. Build positive and trusting relationships with service users, promoting choice, empowerment, and recovery. Provide practical and emotional support to individuals with mental health and wellbeing needs. Support service users to access education, employment, training, social activities, and community resources. Identify barriers to engagement and develop creative strategies to improve participation and outcomes. Provide support during periods of crisis, helping to reduce risks and promote stability. Complete safeguarding referrals where required and ensure concerns are reported appropriately. Assist individuals with maintaining a safe home environment and developing independent living skills.
