Creative Support are looking to recruit a support worker who is experienced in working in with people who have mental health support needs, for our Gateshead based service. You will be working with an outcome-focused floating support team for people over the age of 18 who are experiencing homelessness, or who are at risk of homelessness and have housing related support needs. The service is commissioned by Gateshead Council and will provide a time-limited support service from an office in Gateshead. The service will work with individuals with complex and often chaotic lives to provide a range of support options, including: Support in people's own homes Drop-in sessions and activities/courses at our office Signposting services to ensure people have the support they need to find and maintain tenancies in the local community. We are SIFA Fireside, the support service for any adult in Birmingham that faces homelessness. For 40 years we have supported Birmingham s most vulnerable people through Homelessness Intervention, Prevention and Recovery services. The Adult Lead Worker service is based at our main hub in Digbeth but provides city-wide support to adults experiencing multiple disadvantages. This service provides flexible, strengths-based advice and support to people aged 18 years plus who experience two or more of substance use, poor mental health, history of repeat homelessness and offending history. Clients may also be serially excluded from services or be at risk of exclusion from mainstream services. Key Responsibilities Manage a full caseload of service users as a named key worker, supporting their recovery journey Carry out comprehensive assessments, develop care plans and coordinate onward referrals Deliver psycho-social interventions, including CBT-informed, solution-focused and motivational approaches Provide harm reduction advice and education, including safer drug use, safer sex and blood borne viruses (HIV, Hepatitis A, B & C) Support individuals prescribed Opioid Substitution Therapy (OST) and other treatment pathways Assist nurse prescribers during prescribing clinics and coordinate appointments and care Facilitate and deliver group work interventions to support engagement and recovery Deliver or support needle exchange services and wider harm minimisation initiatives Work collaboratively with multi-disciplinary teams and partner agencies to meet safeguarding and mental health needs Maintain accurate, high-quality clinical records and support service user involvement throughout care Essential Requirements Prison Clearance Proven experience working within a substance misuse/drug & alcohol service, particularly with opiate dependency Experience working within a structured treatment system, including OST pathways Strong understanding of harm reduction principles and interventions Experience managing a caseload and delivering structured interventions Ability to carry out assessments, risk management and recovery planning Confident delivering psycho-social interventions and group work Excellent communication skills and ability to engage hard-to-reach or complex clients Ability to work effectively within a multi-disciplinary and multi-agency environment If you are an experienced Recovery Worker looking for your next interim opportunity within a specialised setting, we would love to hear from you.
