You will be joining an outstanding set-up within a well-known Community Rehabilitation Team in Forfar. Please note that due to the community nature of the role, you will be required to be a car driver and owner. The hourly pay rate for this role is £30 - £40 P/H. This role will involve working flexibly across Orthopaedics and Medical wards, supporting safe and effective patient discharges and promoting functional independence. Key Responsibilities Undertake comprehensive OT assessments for patients within acute and inpatient settings. Plan and deliver patient-centred treatment and rehabilitation programmes. Contribute to discharge planning, ensuring safe transition home or to community settings. Provide expert advice and education to patients, families, and multidisciplinary teams. Support prevention of hospital admissions through timely intervention and assessment. Maintain accurate and timely clinical documentation in line with HCPC and Trust standards.
