The client is currently highly dependent on her husband for all aspects of her daily living. The aim of the support package is to improve the client's quality of life, and support her in becoming more independent. The support worker will be required to complete detailed daily logs on 'Log My Care', and work collaboratively with the multidisciplinary team (MDT) to support and implement therapy goals. Client has a prosthetic limb, and also uses a wheel chair. We are looking for enthusiastic and motivated relief support workers to join our team providing support to people living in our supported housing development in Longwood, Huddersfield. Our service in Calderdale has recently been assessed by CQC as 'Outstanding' at being 'responsive to meeting people's needs and helping them achieve and celebrate goals relating to their health, independence and social lives'. You will be required to work effectively as part of a team to provide person centred support in a consistent manner to promote independence, inclusion, wellbeing and personal development. You must be motivated to provide personalised support based around activities that include; daily living tasks such as cooking, cleaning, laundry; supporting people to meet their personal care needs; and enabling people to safely access their local community. This is a Relief Support Worker position which enables you to pick up shifts at a time that suits you, which is ideal for fitting around personal commitments, study or caring responsibilities. Twelve months minimum of previous experience within a care role is an essential requirement for this relief based role, successful applicants must attend mandatory induction training courses to begin this role.
