The aim of the support package is to improve the client's quality of life, and support her in becoming more independent. The support worker will be required to complete detailed daily logs on 'Log My Care', and work collaboratively with the multidisciplinary team (MDT) to support and implement therapy goals. Client has a prosthetic limb, and also uses a wheel chair.
