At our Ashley Road project, you ll work alongside people who have experienced homelessness, trauma and multiple disadvantage, offering the consistency, practical support and belief that can help them begin moving forward. As a Project Worker, you ll hold a caseload and build purposeful, trusting relationships with clients. You ll complete assessments, develop person-centred support and safety plans, and help people identify goals that feel meaningful and achievable to them. This could include supporting someone to improve their health, address substance use, manage their accommodation, strengthen their daily living skills or reconnect with services and opportunities in their community. The working rota for this post is Monday - Friday, with no weekend shifts . The rota will consist of a mixture of earlies (8am - 3.30pm) & lates (3pm - 10.30pm). About you: You know how to meet people where they are, building trust with people who may be facing homelessness, mental health challenges, substance use, trauma or other complex needs. You bring patience, creativity and warmth to your work, especially when someone has been let down by services before or needs time to feel safe accepting support. You re confident holding a caseload, but you never lose sight of the person behind the support plan their strengths, choices, goals and hopes for the future. You understand that progress rarely happens through one service alone, and you re ready to work closely with health, social care, housing and criminal justice partners to help people get the right support. About the Assertive Outreach and Transitions Worker role: Too many women are lost in the gap between being offered accommodation and feeling safe enough to accept it, trust it and stay. As our new Assertive Outreach and Transitions Worker, you ll stand alongside women at this critical point, making sure a move away from rough sleeping feels possible, supported and led by their choices. Working across the streets, community settings and our Women s Recovery Service, you ll begin building relationships with women before they move in. You ll carry out flexible, trauma-informed assessments, help remove barriers to accommodation and provide steady, practical support throughout the transition into the service. You ll also hold a small caseload of women who need a more flexible or assertive approach to remain connected, use their bedspace and sustain their accommodation. About the Night Manager role: At Camden Young Person s High Support Accommodation Service, nights matter. They can be the point where worry, distress, conflict or risk comes to the surface - and where calm, skilled leadership can make all the difference. You will mentor support workers, leading by positive example alongside having your own case load. You will be responsible for providing person-centred support to clients with serious mental illness including adults who have a primary diagnosis of a psychotic illness often with one or more coexisting conditions such physical health issues, neurodiversity, drugs & alcohol use and cognitive impairment. Lead shifts, delegating set diarised and daily tasks Take the lead during incidents, safeguarding and complaints in the absence of the manager, ensuring policies and procedures are adhered. Ensure all staff are completing relevant health and safety tasks Mentor new support workers through induction including the completion of the Care Certificate and medication training. Complete the care certificate assessors training. Work within a One Team Approach and ensure this culture permiates through out the team and lead by example. Undertake assessments of referrals and attend regular assessment and risk management training. Provide emotional support, encouragement, and practical assistance to residents with daily living activities. This may include medication assistance, engaging with interests, education, accessing community groups/facilities, employment, teaching life skills such as shopping, budgeting, using public transport and skills such as planning to prepare and cook meals, maintain accommodation. Have good knowledge of how to support with referrals and assessments An understanding of equality, diversity and inclusion issues impacting people s lives. IT literate with experience using MS Office packages. Previous related work experience. Good communication skills, written, verbal and listening.
