Administering shared ownership applications, staircasing and voluntary/right to acquire sales. Managing data entry for properties and buyers across various platforms. About the Homeless Pathway Coordinator role: No one should leave hospital with nowhere safe to go. As Homeless Pathway Coordinator, you ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision. Day to day, you ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services.
