Preparing for Adulthood (PfA) Coordinators manage the Education, Health and Care (EHC) Plan process to ensure that young people with SEND aged 14-25 receive appropriate assessments, services, and provision. Coordinators are responsible for a caseload as defined by the Service Manager and have prime responsibility for coordinating the entire process with families to create an EHC Plan. This will include presenting requests to the appropriate Panels, coordinating assessments from other agencies, and drafting and finalising the plan with families. As part of this process the coordinator will be expected to act as a professional exemplar in carrying out all duties with a 'can do' attitude and continue to improve customer satisfaction levels for the service. The coordinator will be expected to undertake duties in an efficient and timely manner, reaching appropriate placement decisions, including being responsible for managing the arrangements for Phase Transfer and transitions through liaison with Social Care and Health services, schools/Colleges, and parents/carers. Coordinators will work closely with service providers and families in order to ensure that the provision outlined in the EHC Plans is being effectively delivered and to take appropriate action where this is not the case. To support this, coordinators will be expected to manage processes including Annual Reviews, and carry forward any requests to change parts of the EHCP. Coordinators will lead Annual Reviews as required and input into monitoring systems for the management of all Review processes. Complex Needs Teacher required for a Specialist School TLTP are recruiting for a Complex Needs Teacher to teach in a specialist school in Uxbridge. This specialist school is for students aged 7-19 with complex needs, PMLD and/or other additional needs. Situated in Uxbridge, they are a small, inclusive school catering for a range of SEN needs. This will involve planning and delivering highly individualised, engaging, and meaningful lessons that consider cognitive, communication, and sensory profiles to low level learners. Work collaboratively with therapists and other professionals to embed therapeutic strategies into daily teaching. Use augmentative and alternative communication systems (AAC) including high-tech (e.g., communication devices/apps) and low-tech systems (e.g., PECS, Sign-Along, visual supports). Build strong relationships with families and contribute positively to home-school communication. Play an active role in annual reviews and contribute meaningfully to Education, Health, and Care Plan (EHCP) processes. As a result, their pupils thrive; they achieve a variety of qualifications and develop into confident, articulate, and ambitious young adults who are curious to further their knowledge and stretch themselves. To apply you will need to have QTS or QTLS and some SEN experience ideally. The teacher will be responsible for the progress of all pupils in their specialism. The teacher will be responsible for coordinating the work of a team of SEN TAs as well as providing required direct teaching time to meet the needs of the class groups and pupils.
