Job Title: Care Coordinator
Salary: Up to 32,000 PA DOE
Location: South London
This is an excellent opportunity for an experienced care coordinator to join a leading independent home care provider within their South London branch and support the delivery of personalised, high-quality self-funded care.
Our client deliver up to 2000 hours of care per week, and have recently been inspected as Outstanding by CQC.
Requirements
- Ideally NVQ Level 2 or 3
- Previous experience within a scheduling role
- Knowledge of CQC compliance is essential
- Good working knowledge of IT Systems
- Ideally experienced in conducting care assessments, care reviews and being able to motivate care staff communicate effectively and efficiently via phone, email and in-person meeting
Responsibilities
- Day to day management of schedules around caregivers and clients.
- Working with clients and their families
- Ensure care delivered is compliant with all relevant legislation
- Undertake assessments and reviews
- Undertake supervisions and direct observations
- Improving and maintaining the continuity of care
Remuneration and Benefits:
- Up to 30,000 PA DOE
