About the Role As part of the Hospital Discharge Team, you'll help facilitate timely, safe discharges from acute settings - ensuring the right care and support is in place from day one. You will: Manage a caseload of adults with complex needs Carry out strengths-based assessments and create tailored care and support plans Deliver statutory responsibilities, including Care Act assessments, reviews, and safeguarding interventions Work closely with health professionals, therapy teams, discharge coordinators, and care providers Prioritise early intervention, recovery, and independence Participate in a duty rota (1 week in every 4) This is an excellent opportunity to make a tangible impact at a critical point in service users' lives, while also enjoying a collaborative team environment and regular professional supervision.
