Adults Social Worker- ARCH- Hospital

Our client, Stockport Council, is seeking an Adults Social Worker to join their ARCH Hospital Team.

Responsibilities of this role:

• Work within a multi-disciplinary team (MDT) to support complex cases and hospital discharges.
• Complete Care Act assessments, mental capacity assessments, and safeguarding work.
• Champion a positive patient journey, striving for least restrictive outcomes and supporting people to live "Gloriously Ordinary" lives.
• Work at pace to support hospital pressures, always keeping the person at the heart of decisions and upholding our Home First values.
• Drive and support the priorities of Stockport Social Care, embracing change management, transformation, and statutory compliance.

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have minimum of 3 years post qualifying social work experience
